VISTA :

Visit Control Panel (viewed control panel home, not classic view)
* Under Programs, select “Change Startup Programs”
* When Windows Defender pops up, scroll down to CtHelper Application (CtHelper.exe) and disable any entries regarding it. (by clicking disable button - there may be more than one CtHelper present too! Maybe disable all Creative start-up entries…
* Go to Creative Audio Console & Set your speakers to 5.1 or whatever, then restart to test it.

Your Center Speaker and Surround may now function with reduced volume. If this is the case, do the following:* Visit Control Panal (Classic View) and find “Sounds”
* Select “Speakers - Creative X-Fi / Audigy” and hit the “Properties” button
* Navigate to “Levels” and hit the “Balance” button
* Here you will find independant volume levels for your Creative soundcard

XP : to fix 5.1 issues you can type “msconfig” in a run command. Choose start-up tab and disable CThelper.exe there.

It's issues like this among many other things that put me off purchasing a creative x-fi. Instead I'm looking at the Asus Xonar lineup.

Which creative X-FI model are you using?

I am using an XtremeMusic too , I just installed the latest drivers on top of the current drivers , there was one issue which always plagued my system and still does even with the latest drivers that is stuttering in vista 64, but in XP x64 everything super no stuttering what so ever, dont know whether this is a problem with vista or creative drivers.

one more thing I have logitechs z5500 digital speakers and I am using an optical out from my sound card to my speakers, I have no issues everytime i reboot my settings stay. Maybe this is a vista problem for you.
