I use the following setup:
Audio:
MP3 --->Winamp 5 w/ Shibatch mpg123 24bit mp3 decoder plugin & out_ds_ssrc.dll
Other audio formats:
FLAC and SHN --->Winamp 5
My soundcard is a Philips PSC706 5.1 16bit/48khz card that is quite old now but still sounds better than most newer cards and manages to play most new games without a hitch.
Video:
MPEG/DiVX/etc (any video file) --->Winamp 5
DVD movie --->PowerDVD 7.0
TV --->SageTV 5.0
Ripping --->Exact Audio Copy + LAME MP3 (settings: VBR avg.bitrate=256KBPS --nspsytune --lowpass 19.3 -q 0)
I really find no problem with Winamp 5.... if you use it with the right mp3 decoder, output plugin, and remove all junk (Ripping/Burning/Modern Skins/Media Library and the DSP/General Purpose plugins). I basically have only the DS Output plugin, necessary input plugins, and the Milkdrop vis plugin which kicks serious butt.
PowerDVD 7.0 is the only DVD player that hasn't crapped out on me so far this year.
I know there are newer formats, but I still use MP3 ~256KBPS vbr because it sounds great at higher bitrates, has some of the best support with 3rd part plugins to improve quality, uses virtually no cpu power, and has the best portable hardware support. For my purposes, EAC and LAME work perfectly.
And if you have a TV Tuner, try SageTV 5.0, very very awesome program.
As far as worsts go:
Windows Media Player (classic or newer) sucks in all dimensions.
Quicktime sucks
RealAudio sucks
WMV sucks
WMA sucks
